- Currently, we DO NOT have a contract with any MEDICAID Plans.
- Some services may require insurance pre-authorization to be covered.

FEES
NO SHOW / CANCELLATION Within 24 hours – $100 Intake, $50 Follow-Up
Documents/Letters – $35 (per page)
Paperwork/FMLA/Applications – $35 (per page)
Medical Records Request (Coordination of Care with Provider/Facility/Hospital/Insurance) – NO CHARGE
Medical Records Request (Self/Guardian, Law Firm, Disability Agency, Other) – $40 for the initial 100 pages, then $0.10 per page thereafter.
Prior to completing any DISABILITY PAPERWORK, the patient must understand the following:
- Must be an established patient (greater than 6 months), we do not complete disability paperwork for New Patients.
- Our providers DO NOT complete long-term disability paperwork.
- Our providers DO NOT complete SMI paperwork.
Note:
- We DO NOT offer sliding scale payments.
- The cash rate reflects the cost of services/treatment/fees not covered by insurance for whatever reason.
- Fees are subject to change without notice.
- Telehealth visits not covered by insurance will be subject to the cash pay rate.
- Your Credit Card information will be required prior to receiving any services.
- In accordance with the No Surprises Act, we will make a good-faith effort to notify patients when their services exceed $400.
